Operations Audit Checklist for Founder-Led Businesses

Short answer: Audit your operation across five areas: process (what happens and who owns it), data (where it lives and how many copies exist), tools (what's used and what overlaps), people (what's manual and what's single-person dependent), and risk (what breaks if someone leaves). Work through each with the questions below and rank findings by frequency multiplied by pain.

Section 1: Process

• Can you list your core processes on one page? Name them.
• For each, what is the trigger and what is the definition of done?
• Who owns each process end-to-end — one named person?
• Where does work wait? Identify every handoff.
• Which processes have undocumented exceptions that happen more than monthly?
• Which steps exist only because of a limitation you've since removed?

Section 2: Data

• List every data type you hold: clients, contacts, deals, projects, invoices, documents.
• For each, where does it live? List every location, not the intended one.
• Which location is authoritative? Is that written down anywhere?
• How many times is the same fact entered by a human?
• When two systems disagree, what's the resolution process?
• How current is the data in your CRM? Check twenty records at random.

Section 3: Tools

• List every piece of software you pay for, with cost and named owner.
• Which are actually used weekly? Check the logins, not the assumptions.
• Where do two tools do the same job?
• Which pairs of tools are connected by a human rather than an integration?
• Which tools hold data you'd struggle to export?
• What did you buy in the last eighteen months that hasn't changed anything?

Section 4: People and manual effort

• What does each person do manually because systems don't connect?
• Which tasks can only be done by one person? List them by name.
• What questions does the team ask you that a document should answer?
• How long does a new starter take to become productive?
• What personal spreadsheets and side systems exist? Ask directly and without judgement — these are your best diagnostic.

Section 5: Risk and continuity

• What breaks if your longest-serving operations person leaves tomorrow?
• Which processes have no documentation at all?
• Where is client data stored, and do you have a record of it? (This is also a GDPR question.)
• Which AI tools are in use, by whom, on what data? Most businesses underestimate this.
• What's your backup and access-recovery position for each critical system?

How to prioritise the findings

Score each finding on frequency (how often it bites) and pain (time, money, or risk when it does). Multiply. Then sort into three tiers:

• Fix now — high score, low effort. Usually two or three items, often solvable in a week.
• Design properly — high score, high effort. These become the rebuild scope.
• Leave alone — low score. Resist the urge to tidy these. Perfectly good operations have rough edges.

What to do with the output

Turn the findings into a one-page current-state map and a ranked roadmap. Share both with the team — they'll correct things you got wrong, and their corrections are usually the most valuable part of the exercise.
